Alt Tab sometimes shows duplicate windows, or doesn't show a window

Describe the bug

I've run into this a couple times, where ALT TAB will show a duplicate window for an app. It usually happens with my DAWs (Reaper and Logic Pro), where it will display the app window, and then a 2nd window with just the icon (see screenshot). If you click the duplicate icon, it takes you to the app like normal.

I've also had it where sometimes minimised apps don't show up in the alt tab switcher (such as my browser), but I can't seem to reproduce that at the moment.

Could you please share a video when this happens?
I'd like to see the Dock. I wonder if it could be possible that there are 2 apps running actually: 1 has an open window, and the other doesn't.

I'm suspecting this because on the right we see the app icon. AltTab shows this when an app has no open window. Yet there is an open window on the left. One explanation could be that there are 2 similar apps. Some apps do this where they have a helper app / assistant. So there are multiple apps technically at the same time.

If not that, then I'm not sure. A video would help to understand how this Logic Pro app works, how the windows interact, etc.

I'll get a video to you shortly, but in short, with Logic Pro there's only one window open. The only possibility I could think of for 2 apps running would be the AU Hosting Service (which runs Logic's plugins), but that's a service, with no ui. I tried the latest release, same problem.

Could you please do the following?

Run AltTab from Terminal.app with this command: /Applications/AltTab.app/Contents/MacOS/AltTab --logs=debug
Show the Terminal windows with the scrolling logs
Launch Logic Pro while showing the Terminal on-screen so we can see the events it triggers
Share that video and the logs here

Interestingly I've only noticed it with DAWs (Reaper and Logic), and some weirdness with my web browser (Firefox), where it sometimes actually doesn't show the open window at all.
